What we collect: name, age, contact info, emergency contact, health history, and your photo and video release choice.

How it's used: Michael and your coach use it to build a training program that's safe for you.

While you're filling it out: your answers are saved automatically in your own browser on your own device, so an accidental refresh doesn't erase your progress. That draft never leaves your device and is never seen by us unless you press Submit.

How it's stored: Your questionnaire is emailed to Michael and printed. The paper copy is filed alphabetically in a locked filing cabinet inside the Trinity Training building. The email copy is kept as a backup.

Who sees it: Trinity Training coaching staff only.

How it's used: to answer your question or follow up on your sign-up.

Delivery: Messages are delivered to Michael's email through Resend, a form-delivery service. Resend does not use your information for its own marketing.

We only use photos or videos of you (or your child) in marketing if you said yes on the Health Questionnaire's Photo & Video Release. You can change your answer anytime by contacting Michael.
